 Job Description 
 Responsibilities of the Clinical Documentation Specialist include: 
 Facilitating improvement in the overall quality, completeness and accuracy of medical record documentation 
 Obtaining appropriate clinical documentation through extensive interaction with physicians, nursing staff, other care team members, and medical records coding staff 
 Educating all members of care team on documentation guidelines on an ongoing basis 
 Demonstrating knowledge and skills necess!   ary to provide care appropriate to the age of the patients ser!   ved 
 Demonstrating knowledge of the principles of growth and development over time 
 Maintaining quality admission reviews 
 Completing follow-up of admission reviews 
 Supporting the growth and development of care team members 
 Participating in performance improvement activities 
 Providing care that reflects an understanding of individual cultural and religious diversities and needs as well as recognition of patient rights 
 Required Skills 
 4+ years of clinical experience 
 3-5 years of acute care experience 
 2+ years of case management or performance improvement experience 
 Knowledge of discharge planning, ICD-9 coding, utilization review, quality management, data analysis, managed care, and management 
 Computer proficiency 
 Valid New York State Registered Nurse (RN) licensure 
 Bachelorâs degree in Nursing (BSN)
