Provides proper individual patient care under the direction of the Nurse team leader.

ESSENTIAL! FUNCTIONS

Each Employee consistently demonstrates the following essential functions:

Supports families, directs them to appropriate professional staff.

Upholds the principles and practices of primary care.

Provides and/or assists the resident in bathing and all other ADL's as per care plan.

Maintains appropriate toileting/bowel regimens as per individualized care planning.

Assists with feeding residents, distributes trays, nourishments and fluids and records intake appropriately.

Responds to resident requests in a timely manner.

Provides designated exercises and repositioning for the residents.

Receives/gives report to team members/team leaders at the stipulated time with all information compiled.

Assists in maintaining resident equipment and environment in a neat clean manner, assuring process to repair same is followed.

Documents ADL's appropriately in Care Tracker.
Provides resident and family emotional support.

Act! ive involvement in Care Plan process

Skin evaluation and reporting.

Transports to and from activities as needed.

Provides leisure activities as needed.

QUALIFICATIONS

Education
High school diploma or equivalent.
Ability to read, write and speak English, ability to follow verbal and written instruction.

Experience
Completion of NYS approved certified nursing assistant program.

Required Certification/Licensure
CNA registered in NYS.

Physical Demands
Continuous standing and/or walking.
Frequent bending and twisting of neck and back, reaching below waist level, squatting.
Occasional pushing/pulling, lifting portion of patients’ weight up to 35 lbs; over 35 lbs requires! assistive devices or other employee assistance.
Fine manual dexterity for computer use.
