identifying patients requiring social work intervention. Performing assessments, developing and implementing treatment an! d discharge plans. Coordinating complex discharges to appropriate care settings involving families, insurance companies and outside facilities and services. Serving as liaison with community agencies and referral services. Formally and informally, educating and teaching other health professionals through conferences and workshops, etc. Preparing necessary documentation in medical records, preparing statistical reports. Complying with required standards for medical/legal reporting situations when necessary. Respond appropriately to professional staff and patient demands.
Assisting management in sustaining a high quality of service.
REQUIREMENTS:
M.S.W. from an accredited School of Social Work
Current licensure in NY State.
Must have 3-5 years licensed, Social Work experience in a medical and/or psychiatric hospital setting and IPRO Screen training preferred.
Bilingual skills a plus.
