2 years minimum case manager experience in a hospital setting. CM cert preferred along with PRI cert NY exp. Goal for caseload is 1:20 pts. Maybe be overload depending on staffing & total scheduled discharges. Discharge planning is part of this role. Generally work M-F w/ 1 weekend per month. Definitely Saturday, but Sunday will be dependent on census. Shift time is 8a 4p or 9a 5p Canopy and ECIN as our case management systems (MA director). ST56
